The Obama administration undertook massive expansion of US spying without reviewing the weaknesses in the system that allowed people like Snowden and Manning to access extraordinary amounts of important data.

Yeah, from statist neocon Meade, that’s the primary flaw:  When the spymasters decided to turn America into an ubiquitous surveillance state, they didn’t take enough precautions to prevent people like Snowden and Manning from exposing what they were up to.
